Enrollment Requirements
At Community Learning Academy, we follow all state guidelines to ensure a smooth enrollment process for every student. Families can review the requirements below, which include documentation of age, residence, health records, immunizations, attendance, and permanent student records, in accordance with Florida law.
Florida Student Enrollment Requirements
When enrolling a student in a Florida K-12 school, the following documents and actions are generally required under Florida law:
Proof of Age / Birth Certificate
Students must provide an official document showing their date of birth (such as a birth certificate or passport). Florida Department of Health in Orange+1
Proof of Legal Residence
Documentation showing the parent(s)/guardian(s) live in the appropriate school district or county. Florida Department of Health in Orange
Health Examination
A completed physical examination is required within 12 months prior to the child’s school entry date. The school‐entry health exam form is usually required (often called DH 3040 in Florida). Florida Department of Health in Orange+1
Immunization Records
Students must present a Florida Certification of Immunization (Form DH 680) or an approved exemption. All immunizations required by the Florida Department of Health must be up to date. Florida Department of Health in Orange+1
Permanent Enrollment Records / Student Records
Schools require certain student records, which may include transcripts, records from the last school attended, full name, date of birth, address, parents’ names, attendance, courses taken, etc. In the case of private schools, permanent student‐records are important especially if transferring or finalizing a diploma. Attendance Policies and Compulsory Schooling

Other Health & Safety / School Regulations

Schools must comply with health and safety inspections, sanitation, food service (if applicable), fire code, etc. Florida Department of Education+1
